- Certificate of Title Bond Form 78-016-02-1-1-000 with an accompanying power of attorney issued by a
licensed Mississippi agent. This form is supplied to you by us when you purchase a bond. This form
will be completed and only needs your signature.
- An Affidavit of Ownership of A Motor Vehicle For Which Application For Title Under Bond Form 79-013-
02-1-1-000. This form is supplied to you by us, however, you will need to complete this form and have it
notarized.
- A Bill of Sale. The bill of sale should include the seller's name, address, phone number, the vehicle
year make, body style, and identification number and the buyer's name, address and phone number.
- Form MVT-59. The vehicle must be taken to the nearest Mississippi Department of Public Safety
substation for inspection and issuance of Form MVT-59 by the inspecting officer prior to making
application for certificate of title.
